Sultamicillin (tab: tosylate)
Indications/Uses
Upper resp tract infections including sinusitis, otitis media & tonsillitis; lower resp tract infections including bacterial pneumonia & bronchitis; UTI & pyelonephritis; skin & soft tissue, intra-abdominal & gonococcal infections. Patients requiring sulbactam/ampicillin therapy following initial treatment w/ sulbactam/ampicillin IM/IV.
Dosage/Direction for Use
Adult including elderly 375-750 mg bid. Uncomplicated gonorrhea 2.25 g as a single dose w/ concomitant probenecid 1 g to prolong plasma conc of sulbactam/ampicillin. Infection caused by hemolytic Strep Administer for at least 10 days to prevent occurrence of acute rheumatic fever or glomerulonephritis. Childn ≥30 kg Usual adult dose, <30 kg 25-50 mg/kg daily in 2 divided doses. For both adult & childn, treatment is continued until 48 hr after pyrexia & other abnormal sign have resolved. Duration of treatment: 5-14 days.
Administration
May be taken with or without food.
Contraindications
History of allergic reaction to any of the penicillins.
Special Precautions
Previous hypersensitivity reactions to penicillins, cephalosporins & other allergens. Discontinue if severe skin reactions eg, TEN, SJS, exfoliative dermatitis & erythema multiforme occur. Observe for signs of overgrowth of non-susceptible organisms including fungi; discontinue if superinfection occurs. Clostridium difficile-associated diarrhea. Drug induced liver injury eg, cholestatic hepatitis & jaundice. Not to be used in patients w/ infectious mononucleosis. Periodically check for organ system dysfunction during prolonged therapy including renal, hepatic & hematopoietic systems. Severe renal impairment. Pregnancy. Not recommended during lactation. Neonates. FC tab: Not to be taken by patients w/ rare hereditary problems of galactose intolerance, Lapp-lactase deficiency or glucose-galactose malabsorption. Powd for oral susp: Not to be taken by patients w/ rare hereditary problems of fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency.
Adverse Reactions
Pseudomembranous colitis, candida infection; thrombocytopenia; anaphylactic shock/reaction, Kounis syndrome, hypersensitivity; dizziness, somnolence, sedation, headache; dyspnea; enterocolitis, melena, diarrhea, vomiting, abdominal pain, dyspepsia, nausea, stomatitis, dysgeusia, tongue discoloration; jaundice, abnormal hepatic function; TEN, SJS, erythema multiforme, angioedema, urticaria, dermatitis, rash, pruritus; arthralgia; fatigue, malaise; increased ALT/AST.
Drug Interactions
Increased incidence of rashes w/ allopurinol. Additive effects w/ anticoagulants. Bacteriostatic drugs (chloramphenicol, erythromycin, sulfonamides & tetracyclines) may interfere w/ the bactericidal effect of penicillins. Reduced effectiveness of estrogen-containing OCs. Decreased clearance of MTX. Decreased renal tubular secretion w/ probenecid. False +ve glycosuria in urinalysis using Benedict's/Fehling's reagent & Clinitest.
